• Aetna - currently closed to new patients

Please contact your insurance company to verify coverage prior to scheduling an appointment.

Payment

Private pay patients are offered a "pay at time of service" discount. Payment arrangements can be made, please inquire at the office. Acceptable forms of payment include Visa, Mastercard, checks and cash.

Late cancellations and missed appointments

Please provide 24-hour notice if you need to cancel an appointment. Appointments cancelled without 24-hour notice may incur a fee of $50. New patients who miss their first appointment without 24-hour notice will not be rescheduled. Current patients who miss 2 appointments without 24-hour notice may incur an additional fee and may be discharged from the practice.
